Best Local Social Prescribing Team/Service

Carr Gomm is proud to be part of the Edinburgh Community Link Worker Network, recipient of the award for Best Local Social Prescribing Team/Service.

The 2024 Social Prescribing Awards hosted by the Social Prescribing Network and the National Academy for Social Prescribing recognises and celebrates innovative social prescribing projects and schemes in the UK and internationally.

The Award recognised the impact that the Edinburgh Community Link Worker Network has had in lives of people across Edinburgh since its introduction in 2017. Dawn, one of our Community Link Workers, joined with others to accept the award.

Carr Gomm’s Community Link Working Team and the wider Network offers non-clinical one-to-one support to people in contact with GP practices to keep people well within their communities, both mentally and physically.

Through working with people, our Community Link Workers prevent people from becoming unwell, returning to their GP, or being hospitalised; achieved by addressing the root causes of their stress or distress, whether poor housing, debt, unemployment, or feelings of isolation.

Our Community Link Worker Project works because:

  • Community Link Workers know their community.

They understand the best groups and activities for the person, often going along with the person to support them on their first visit.

  • Community Link Workers get to know people.

They are positively persistent; building relationships and understanding their aspirations to help people access the right support at the right time.

  • Community Link Workers work in partnership.

They are a link between the NHS and third sector, increasing the support offered by GP services and strengthening support networks in the community.
